STEP 2:

Once you have contacted our office to establish care, you will receive a link to complete your intake forms.

STEP 3:

Once all patient forms and insurance documentation has been received and verified, you will receive the details of your initial assessment and medical appointment. Additionally, you will also receive information regarding your group therapy appointments.
