Connecting with your care management team is an essential first step in enrolling in our 1915(i) services, including Individual Support, Individual Care and Support, and Community Living and Support. Your care management team will guide you through every stage of the enrollment process, helping you understand the services available and how they can best meet your unique needs and goals.

They will assist with completing the necessary paperwork, coordinating services, and ensuring you have access to the resources needed for a smooth and successful transition. By partnering closely with your care management team, you can feel confident that you are receiving personalized support designed to promote independence, enhance your well-being, and improve your overall quality of life.
