Community Alternatives Program for Children
CAP/C provides comprehensive home and community-based services for medically fragile children who would otherwise require institutional care.
We partner with families to coordinate individualized services that meet each child's unique developmental and medical needs.

Who Qualifies

Children under age 18 with complex medical needs or disabilities

NC Medicaid enrolled
-
Assessed as requiring nursing-facility-level care
-
Family is willing and able to participate actively in the child's care

Enrolled through Trillium Health Resources (LME/MCO)
